Review comments on Markdown files, stored as plain HTML comment tags in the file itself — no sidecar files, no CriticMarkup, no git refs. Diffs cleanly and renders invisibly everywhere.
Installation
Launch VS Code Quick Open (Ctrl+P), paste the following command, and press enter.
Inline review comments for Markdown files in VS Code.
Add comments directly to your .md files as HTML comment tags — invisible in rendered output, tracked in git, and visible in the custom MD Comment preview panel.
Features
Custom preview panel — split view with rendered markdown on the left and a comments pane on the right
Click to comment — hover any paragraph, heading, list item, or code block in the preview, click + to add a comment
Inline badges — comments render as inline badges in the preview, showing a preview of the text and the date
Right-click shortcut — right-click in a markdown editor → "Add Comment at Cursor"
File tree sidebar — MD Review panel in the activity bar lists all .md files in the workspace; click to open preview
In-file storage — comments live inside your .md file as HTML comment tags, so they travel with the document and appear in git diffs - no sidecar storage involved.
Real-time sync — preview updates automatically as you edit the file
Usage
Open a .md file
Run MD Comment: Open MD Comment Preview from the command palette (Ctrl+Shift+P / Cmd+Shift+P), or click the file in the MD Review activity bar panel
The preview opens to the right, split into a markdown pane and a comments pane
Hover over any block element — a + button appears on the right edge
Click +, type your comment, press Enter
Managing comments
Edit: hover a comment badge or card → click ✏️ → edit in the popover → Enter to save, Escape to discard
Delete: hover a comment badge or card → click 🗑️ (asks for confirmation)
Navigate: click any comment card in the right pane to scroll the preview to that line
Reveal in editor: double-click anywhere in the markdown pane to reveal that line in the source editor
Notes
Comments auto-save immediately — no manual save needed after adding or editing a comment
If the editor has unsaved changes, the comments pane shows a warning before you add a comment (which will save the whole file)
The preview remembers your scroll position across re-renders
Comment Format
Comments are stored as a single HTML comment line in the .md file:
<!-- @comment id="abc123" text="This needs revision" created="2026-04-02" -->
The paragraph being commented on.
This syntax is invisible in all standard markdown renderers (GitHub, Hugo, etc.) and valid in all HTML-based outputs.
