This is Visual Studio extension for opening project dll/exe in ildasm tool. To be able to use it you have to configure path to ildasm.exe. You can do this in Tools -> Options -> DisassemblyMe and select a path. If you want to use it just press right button on project and press 'Build and run disassembler' For now the functionality is limited. Please report all bugs.
