Auto-Expand Visual Studio Regions

Make Visual Studio #regions suck less.

Works for 2019 (v16.01 excluded). To have it working on >16.01, please accept Synchronous autoloading of extensions.

Ported for 2017 Preview 3 Community Edition, untouched since. Untested but should work for all versions below (2010- excluded).

Automatically expand regions when opening a file. Prevent regions from being closed while collapsing other code blocks, and blend the region text into the background, making it less noticable. See Tools->Options->Disable Regions for options.
